WordPress - create a Custom Contact Form that goes to Constant Contact

Hello,

I would like to create a form in my WordPress with the following fields:
- First Name
- Last Name
- URL
- Email Address
- Telephone
- Comments

I am having a difficult time try to figure out what to do.
Could someone please point me in the right direction.

Hi Tom,
I looked at this one, but it seems that it only allows for an email address
and not the other fields I need.

Is there something else I could use?

The form only asks for email address on the "first page".  However, if the user enters their email address in that widget, the widget brings them to your Constant Contact "SignVisitor Signup form" which collects whatever set of data you have decided to collect on that form.  You can learn more about the Site Visitor Signup form at this FAQ:

If that doesn't meet your needs, you have a software development task to create a form which uses the APIs on this site.
